Please read this contract carefully as it is a legally binding document. Initial all paragraphs and complete the last page. All fields are mandatory. If you cannot fill out a section or disagree with the contents of any portion, please contact Bubbles Dog Rescue to discuss. We expect this contract and payment to be completed within 72 hours of this email.

A couple important reminders, IN ADDITION to reading and agreeing the contract in FULL:

1. Please begin training 10 days after the dog adjusts to your home, no later. Here are some training resources for you:

2. Please remember to take off the dog's collar when inside, especially during the night.

3. Please remember we are a resource for you, if there is anything you may need, have questions about, etc. PLEASE do not hesitate to contact us if you have any questions.

4. Please ensure to begin tick and flea and heartworm treatment as soon as possible.

5. Please register your pets microchip and register with your county's animal services.

Terms & Conditions*
- I am adopting the pet described below through Bubbles Dog Rescue. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not allow the pet to be used for purposes of vivisection or experimentation.
- I will provide proper and sufficient, food, water, shelter and kind treatment for the pet at all times. I further agree the dog will not be chained at any time as a means of restraint or be a primarily outside dog.
- I agree that a representative of Bubbles Dog Rescue may make periodic inspections of the pet, my residence and the pet’s living conditions on reasonable notice. I further agree to surrender the pet to Bubbles Dog Rescue's representative if for any reason he/she considers the conditions provided by me for the pet to be unsatisfactory.
- I will keep a collar and identification tag on the dog at all times and will license the dog with the county/city.
- I agree to immediately notify Bubbles Dog Rescue if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
- I agree to provide all necessary medical care to the pet a timely manner. I agree to keep all vaccinations current and ensure the pet receives preventative medication to protect against fleas, ticks & heartworm. Any veterinary problems that may arise, including serious illness requiring emergency care or surgery is my responsibility and I am responsible for all related costs. To the best of our knowledge, Bubbles Dog Rescue is giving you a healthy pet unless otherwise noted.
- I agree to provide obedience training for the pet at my own cost if any behavior issues arise. I agree that if the training is not complete for any pet being returned to us, I will be financially responsible for the cost to complete training. Any behavioral issue (ie. biting, destruction, escapism, fearfulness, dominance) that might lead to abandonment of the animal: I agree to contact an obedience trainer as well as Bubbles Dog Rescue - to follow the recommendations in resolving any issues as an effort to maintain custody of the adopted animal.
- If I decide to not keep the pet, I hereby agree to return the adoptee to Bubbles Dog Rescue ONLY. There is no type of reforming allowed. I agree to give Rescue up to 14 days to secure a foster home and will provide safe and loving care of the Dog until it is returned.
- I agree that the rescue makes no warranty as to the temperament or physical condition of the Adoptee. The Adoptee is being adopted as is. All warranties with regard to Adoptee, including any implied warranty of merchantability and fitness for a particular purpose, where they are applicable, are hereby specifically disclaimed.
- I agree that I may not euthanize the pet except in the case of the pet's terminal illness or injury, or old age accompanied by pain and suffering. Which in that case, the euthanasia must be performed by a licensed veterinarian AND I MUST contact Bubbles Dog Rescue as soon as that is scheduled, PRIOR to the euthanasia.
- I hereby release Bubbles Dog Rescue from any and all liability for personal injury, property damage, legal fees, or veterinary care, or any other expense or liability incurred as a result of this adoption. I agree to indemnify Bubbles Dog Rescue from any and all such claims and to pay, without limitation, any costs related to such injury, damage, or liability, including, in the case of litigation, any attorney’s fees incurred by Bubbles Dog Rescue in its defense.
- In the event that I do not comply with the terms of this Agreement, or the pet is abused or neglected, Bubbles Dog Rescue holds the right to recover the pet from me upon demand, and the pet will be surrendered immediately. In addition, without waiving any of my rights under this Agreement, if I do not surrender the pet to Bubbles Dog Rescue , you will owe us a payment in the amount of $2,500 agreed as just and reasonable liquidated damages to compensate us for costs and expenses in connection with your breach of this Agreement and Bubbles Dog Rescue reserves the rights to reacquire the pet.
- This Agreement (including the attached Voluntary Release/Waiver of Liability) is the entire agreement between me and Bubbles Dog Rescue, and supersedes any prior understandings between me and Bubbles Dog Rescue with respect to the subject matter of this agreement. No modification of this Agreement will be valid unless in writing duly signed by both parties. This Agreement is binding upon me and my heirs, assigns, successors, personal representatives and executors.
- ADOPTERS VOLUNTARY RELEASE / WAIVER OF LIABILITY This Voluntary Release/Waiver of Liability (the “Release”) is made and entered between this undersigned and Bubbles Dog Rescue (“Charity”) effective as of the below-indicated date. The undersigned desires to adopt an animal from Charity, and as an inducement to Charity to agree to the adoption, the undersigned hereby represents, acknowledges and agrees as follows: 1. I acknowledge that, due to the unpredictable nature of animals, certain risks and dangers may occur while caring for an animal, including risks of injury to person and property, and that I have voluntarily agreed to make the adoption contemplated herein with full knowledge of such risks and hereby voluntarily assume all of such risks. 2. I agree that I will not make any claims against, sue or attach any property of Charity, its directors, officers, agents, employees and/or volunteers for any injury or damage caused by any animal(s) adopted from Charity. 3. I hereby voluntarily release, discharge, waive and relinquish any and all actions or causes of action for personal injury, property damage or wrongful death occurring to me or third parties as a result of any action or activity of any animal(s) adopted by me from Charity. It is my intention by this instrument to exempt and relieve charity from ALL liability for personal injury, property damage or wrongful death caused by negligence or otherwise. 4. I hereby agree to hold Charity and its directors, officers, agents and/or employees harmless and indemnify them from and against any liability, claims, judgments or expenses I may incur arising out of my adoption of an animal from Charity and/or my ownership or care of any such animal thereafter. 5. I HAVE CAREFULLY READ THE FOREGOING, AND I UNDERSTAND THAT TOGETHER THEY CONSTITUTE A DISCLAIMER OF LIABILITY BY CHARITY AND A RELEASE OF ALL CLAIMS BY ME. I UNDERSTAND THAT I ASSUME ALL RISKS INHERENT IN MY DECISION TO ADOPT AN ANIMAL FROM CHARITY AND ANY RELATED ACTIVITIES. I VOLUNTARILY SIGN BELOW TO EVIDENCE MY ACCEPTANCE OF THE ABOVE PROVISIONS. 6. If any portion of this Release is held invalid, it is agreed that the balance hereof shall continue in full legal force and effect. 7. This Release is governed by the laws of the State of California applicable to agreements entered into and fully performed therein.
